Regional Family Health Encouraging Medication Reconciliation

October is recognized as “Talk About Your Medicines†month.
The idea is to help enhance communication between patients and healthcare providers regarding safe and appropriate use of medications.Â
Regional Family Health in Manchester is encouraging routine “medication reconciliation.†Nurse Manager Ashley Pasker says this process compares patient medication orders to all of the medications the patient has been taking. It helps to avoid medication errors such as omissions, duplications, dosing errors or drug interactions.
And one of the best ways to communicate with your healthcare team – simply ask questions. Regional Medical Center suggests using a checklist and taking notes to make sure your medication concerns are addressed.
